There is an idea that says “everyone deserves to look better and feel beautiful”. Most people are believed that beauty and physical attractiveness are very important, because a pleasing appearance may reflect a person with positive qualities, and attractive individuals have more rewarding life experiences than unattractive individuals. For these reasons, people seek ways to become more attractive and look better.
Face beauty is one of the factors that has a great impact on a person's self-confidence and happiness. Attention to face beauty and physical attractiveness would increase the self-esteem and self-confidence of individuals.
Studies have also shown that beautiful persons, especially those with facial beauty, have higher chances of getting good jobs with high salaries. Therefore, there are obvious reasons why people want to enhance their facial beauty.
One of the factors that affect face beauty is the person’s nose. The nose is considered the most prominent part of the face. A beautiful nose has defined geometrical features that have a profound role in revealing face beauties. One can say that an ugly nose even hides other beauties of the face. One of the very popular cosmetic surgeries is rhinoplasty, which performs on the nose.
A nose job or rhinoplasty is a surgical process that reshapes your nose. It performs for enhancing the beauty of the nose and produces harmony between the nose and other face organs. Nose job also improves the function of the nose, which is breathing.
The nose is constructed by a bony and cartilaginous structure; its upper portion is bone, and its lower portion is cartilage. As we know, it is all covered by skin. In rhinoplasty, a surgeon can change the bony structure to reshape the nose. Sometimes it is necessary to cut or make changes to the cartilage part. Also, it is possible that all three parts including bone, cartilage, or skin, underwent some changes to reshape the nose structure; thereby enhancing its beauty and function.
Every year, about half a million people perform rhinoplasty; some of them for correcting medical problems and others for enhancing their beauty. We can divide rhinoplasty reasons into two parts: (1) medical problems and (2) cosmetic reasons; those procedures are called reconstructive and cosmetic surgery, respectively.
Medical problems can be
- some birth defects
- deviated nasal septum (a procedure called septoplasty)
- sinus condition
- various traumas (blunt or penetrating traumas)
- nose injuries
- breathing problems
- failed primary rhinoplasty
- making the nose smaller or bigger
- increasing or decreasing the size of nose openings (nostrils)
- straightening the nose bridge
- sharping the nose’s tip
Reshaping the nose depends on the goals of rhinoplasty. The procedure might aim to facilitate breathing or improve appearance. Some people want a wider nose, and some individuals tend to narrow their noses. With age, the tip subunit of the nose loses its normal shape and seems ugly. Importantly, alternation of nose tip shape marks people older than they are. Thus, people want to perform rhinoplasty to reshape the tip subunit of the nose in order to seem younger. Many people prefer resizing nostrils (more wide or narrow). Changing the size of nostrils is usually done by breaking the bone of the nose and rearranging the nose cartilage. Removing the nasal hump and increasing or decreasing the overall size of the nose are other reshaping options for rhinoplasty. Nose reduction (making the nose smaller) can be done by removing some of the bones or cartilages of the nose. Nose augmentation (making the nose larger) may perform by grafting bones from the skull, elbow, or hips and also cartilages from the ears to the nose to make a larger nose. It is also possible to change the angle between the nose and the top lip. In addition, the surgeon should expand or shrink the skin over the nose according to the new shape of the nose, whether it is reduced or augmented.
Like any other surgery, rhinoplasty also has some risks, such as:
- difficult breathing
- pain and swelling
- nerve damage and numbness in and around the nose
- bruise on the nose skin surface
- septal perforation
The most common risks are bleeding, infection, and adverse reaction to the anesthetic drug. Although all these risks are possible, the possibility rate is depending on the patient's overall condition, patient nose structure, before and after surgery caring period, the goal of surgery, method of surgery, experience, and expertise of the surgeon. Your surgeon will tell you about the possible risks related to your conditions. Don’t forget that choosing a good or expert surgeon helps you to reduce these risks.
Before any other action, the first step is meeting the surgeon to discuss the goal and patient’s conditions for rhinoplasty surgery. The surgeon should evaluate and examine the patient and proceed with the process if he/she is a good candidate for rhinoplasty. Talking about the goal of surgery is the most important part of this discussion.
This meeting has two parts: (1) medical history and (2) physical examination. Medical history includes the patient's past history, family history, personal history, and drug and allergy history. Physical examination is also including a complete survey of your body and especially paying attention to the face and nose.
A medical history will help the surgeon to know about the motivation of the patient for the surgery and his/her medical profile. It is the most important part that determines if the patient is a good candidate for rhinoplasty surgery. In this part, many contraindications for this cosmetic surgery should evaluate and rule out. A medical history will also predict the patient's possible risk factors for the surgery.
In addition, a physical exam will help the surgeon to be aware of the patient facial feature and also the nose's overall structure (inside and outside). Thereby, the surgeon can decide what changes should be made. He/she can also predict the future shape of the patient nose based on the nose structure and physical features including the shape of bones, strength of the cartilages, and also the thickness of the skin. The most critical prediction should be about the effect of rhinoplasty surgery on the patient’s breathing.
Another important part of preparation is photographs. A photographer will take photographs of the patient nose from different angles. This is the most exciting part of pre-operative preparation. Assume that this is the last time that the patient watches his/her ugly nose. These photos will use for before and after surgery assessments. Most importantly, these photographs will transfer to a computer for manipulation by given computer software. This software will show what kinds of results can be possible, based on the nose structure.
Rhinoplasty may perform under local anesthesia or general anesthesia. The decision about the type of anesthesia is depending on the complexity of the surgery and the surgeon's preference. The surgeon will choose the most appropriate type of anesthesia for every individual patient.
Local anesthesia is usually with sedation. A local anesthetic drug that injects into nasal tissues and their surroundings, will produce numbness and analgesia in the nasal area. Thus, the patient will not be having any discomfort or pain during the surgery. As rhinoplasty surgery like any other surgery is associated with stress during the surgery, a sedative medication is also injected by intravenous (IV) route. Thereby, the patient will be drowsy, but not fully asleep.
General anesthesia is another option. By injecting or inhaling an anesthetic medication, the patient becomes unconscious during the surgery.
After anesthesia, the procedure will start and the surgeon will change the shape of the nose, including the shape of nasal bones, cartilages, and skin by several methods. Although the procedure may take 1.5 to 3 hours, the duration of the surgery is depending on the goal of surgery, nose structure, type of reshaping (nose reduction or augmentation), type of rhinoplasty surgery, and patient medical profile and history, and also type of anesthesia.
After surgery, the patient will transfer into a recovery room. The recovery room is a special room next to the operation room, where the anesthesiologist and the nurses will monitor the patient closely, until returning to wakefulness. All vital signs and also the condition of the nose wound will check regularly. The staying period in the hospital is also depending on some factors, but it is usually 1 or 2 nights.
What is important for post-operative care, is the method of resting in bed and also taking care of dressing. There are also some instructions about activities, taking showers, food, and medications, that patients should consider.
The most exciting part is seeing the results of the rhinoplasty surgery. It is important to know how the nose looks after surgery. The most important is being satisfied with the results. Even very small changes to the nasal structure can result in a huge difference. Changes are often measured in millimeters, but it is often enough to change the shape of the nose. In rare cases, these small changes are not enough, so the surgeon decides about a second surgery.
