What is fractional laser treatment?

It's a type of laser treatment procedure that is non-invasive and utilizes a device to deliver a laser beam divided into a lot of parts of microscopic treatment areas that target a fraction of the skin at a time, analogous to a photographic image being enhanced or altered pixel by pixel.
The fractional lasers functions are between ablative and non-ablative laser techniques because as it's clear ablative ones effect on the epidermis layer of skin mainly and non-ablative ones affect the dermal layer (mid-layer of skin) but fractional lasers are widely used for both dermal and epidermis layers.
The brand of the first fractional laser was Fraxel® but today a lot of various machines are using fractional technology. There are two different types of fractional lasers which are non-ablative and ablative. Fractional non-ablative ones are methods that leave the tissue intact with an injury that is subsequently removed and repaired by the body.

What is a fractional laser treatment used to treat?

The first point fractional users have to know is that who good candidates are? The patient who have the conditions  below :

• Facial lines and wrinkles (rhytides)
• Sun damage
• Skin pigmentation associated with photo-aging
• Surgical and acne scarring
• Civatte and stretch marks

However one of the most common usages of fractional lasers is the treatment of pigmentation disorders like melasma, one of the side effects after fractional laser treatment is post-inflammatory pigmentation.
In comparison to other laser methods, fractional lasers can be widely used in any part of the body ( mostly suitable for neck, chest, and hands areas ).
Although the suggested technique of laser treatments vary on different factors like patient age, skin type, sun exposure, and body location, fractional lasers can be suitable for all skin types.
Like all cosmetic procedures, this type of laser therapy can be combined with other surgical and nonsurgical treatments.

How does fractional laser treatment work?

Before explaining the function of fractional laser it is better to know brief information
about facial anatomy. The skin has three functional layers epidermis (uppermost layer), dermis (mid-layer), and subcutis (lower fat layer). Pigment production (reason for skin coloring) happens in the epidermis layer by special cells which are called melanocytes. The bellowing layer is called dermis and is responsible for collagen and elastin fibers production and these fibers cause strength, toughness, elasticity, and pliability of skin.
The aging process affects skin structure for example epidermis layer will become thinner and blemishes become visible and in the dermis layer, collagen production becomes very less than the amounts it should be. Due to these reasons formation of facial lines, sagging skin, and wrinkles will occur.
Fractional laser treatment works on both two layers, the epidermis layer, and dermis layer. It does this by delivering a laser beam that is divided into lots of tiny but deep columns of treatment into the skin. These columns are called microthermal treatment zones (MTZs).
Epidermal pigmented cells are expelled and the laser also stimulates collagen production in the dermis layer. This fractional treatment results in a faster healing process than if all tissue in the treatment area was exposed to the laser.

Post-treatment and recovery

• The gliding gel is washed off after the fractional laser treatment

• Patients may experience a mild degree of sunburn sensation for about an hour after having the fractional laser procedure

• Swelling in treated  areas is usually minimal and should resolve 2–3 days following the treatment

• The skin will have a pinkish tone at least for the first 3–5 days

• Within 24 hours new epidermal skin develops and the skin will have a bronzed or brown appearance that can last 3–14 days following your procedure. Flaking of the skin may also happen as new skin replaces dead skin tissue, which can be treated with a moisturizing cream

• Skin peeling can be pronounced after ablative therapy when the skin should be gently removed with tap water, petrolatum and gauze once or twice daily.

• During the healing phase and for several months following your fractional laser treatment, it is recommended that the treatment area is protected using a moisturizing sunscreen with an SPF of at least 50+. Protective clothing and wide-brimmed hats should also be used to protect the skin damages from sun exposure

Side effects of fractional Lasers

Some of the side effects and complications that may occur after aggressive or ablative fractional laser treatments, particularly on neck skin are :

• Excessive desquamation or some crusting
• Swelling for up to one week after treatment in treated areas
• Postinflammatory pigmentation (more common in patients of darker skin types)
• Acneform eruptions, herpes simplex, bacterial and candida infections
• Contact dermatitis

The cost of fractional laser treatments

The costs depend on each patient's skin condition, type of treatment, number of necessary sessions, treatment regions but averagely it ranges from $500 to $5,000. The average cost in NYC, according to researches, is $1,500.
