The importance of the beauty

"It's never too early to start being concerned about your appearance – first impressions are everything." Robert Parish said. Now you think, do you want to have a harmonious image? Or are you okay with an ordinary one? If you wish to be perfect you can read this essay to be informed about one of the cosmetic surgeries that you may need.

Look at the mirror. Does your jaw appear to be masculine, too wide, or asymmetrical? If yes, you should book a jaw reduction surgery to make a v-shaped jaw instead of your wide or square-shaped jaw. The jaw has different shapes in men and women. In men because of larger muscles such as masseter, the jaw is square-shaped. But in women jaw is more delicate and slender. Sometimes a woman has a wide jaw, which may be caused by different things.

What can cause a wide jaw?

Wide mandibular bone or large masseter muscle can cause a wide lower face. Each of them can be treated by different methods. To reduce muscle size we can use botox injection, but for reducing the size of the bone we need to perform a surgery named jaw reduction surgery.

Botox injection

The masseter muscle may be large because of genetic or acquired reasons. The more you use this muscle the more its size gets. For instance, if you chew gums repeatedly or clench your teeth, your muscle may get larger.
If you want to treat the enlargement you can use botox injection to make the muscle smaller (atrophy). The weakening can take several months, so others may not notice your changes for a couple of weeks or even months. Actually, in many patients, the most observable changes will be seen after one year, therefore you should be patient with your appearance.

surgery method

Jaw reduction surgery is a part of a facial feminization surgery (FFS) plan in which the size of a mandibular bone is reduced. This surgery is also known as jaw shaving or jawline surgery which the terminology word is mandibuloplasty. It's a major procedure with some risks like other types of surgeries.

Who are the candidates for jaw reduction surgery?

In some countries such as ours, "Iran", the culture usually considers v-shaped chin as a beauty factor in women. In these circumstances having a wide lower face may not be appropriate. So we only in these countries can be candidates for jaw reduction surgery. People who are candidates for the surgery:

  • Are unhappy with their appearance.
  • Don't smoke.
  • Have healthy and active lifestyles.
  • Don't have previous blood or autoimmune conditions.
  • Are willing to undergo a jaw reduction surgery.
  • Have realistic expectations.

Consultation

If you are a candidate for jaw reduction surgery, you should talk to your surgeon during the consultation and decide whether or not this surgery can accomplish your desires. In the consultation you should:

  1. Discuss your expectations very clearly.
  2. If you smoke, stop it at least 3 weeks before the surgery.
  3. Avoid aspirin and other analgesics for 2 weeks before the surgery.
  4. If your surgery is FFS, you need to avoid hormones for weeks before and after the surgery.
  5. If your surgeon wants, you may need to place braces on your teeth about a year and a half before the surgery.

Then you can ask the surgeon:

  • Has she or he done jaw reduction surgery before?
  • How can this surgery be done?
  • Which type of anesthesia will be used?
  • How can I prepare for the surgery?
  • How much does the surgery cost?
  • What should I expect after the surgery?
  • What are the pros and cons?
  • What are the possible risks during jaw reduction surgery?
  • What other options do I have?

The surgeon may also:

  • Take photographs of your jaw
  • Show you what how will you like after the surgery using special devices
  • Show you before and after pictures
  • Measure the size of the jawbone and chin

How much does a jaw reduction surgery cost?

Jaw reduction surgery costs at least 10000$ which are about 20 or 30 million tomans. This payment is only for the surgery itself. You should pay for other factors as well. These factors are anesthesia fees, hospital costs, medical tests, prescription for medications, etc. Remember to read your health insurance carefully. If your surgery is performed to treat functional problems, it may be covered by your health insurance. But if you want to have a jaw reduction surgery as a feminization facility surgery, you should pay for it yourself.

Preparing for jaw reduction surgery

Now that you decide to do the jaw reduction surgery, you need to get prepared for this major surgery. The surgeon usually performs the surgery in a hospital, but depending on the procedure he or she may perform the surgery in an office-based surgical facility, or an ambulatory surgical center. You need to prepare for staying in hospital after the surgery for 2 or 4 days so pack a bag of requirements. If your surgeon doesn't want you to stay overnight you better arrange for someone to pick you up after surgery.

Procedure steps

1. Anesthesia
The most common type of anesthesia in jaw reduction surgery is general anesthesia in which you are completely unconscious and pain-free.
2. Incision
Most of the time the surgeon makes an incision inside the mouth so that the scars will be invisible after the surgery. The incision is made between your gum and cheek to have better access to the mandibular bone. The surgeon must be very careful about the incision to avoid infection and inflammation later.
3. Shaving the jawbone
After the incision, the surgeon will shape marked areas of the mandibular bone to achieve a more desirable, delicate contour. He or she can do that by shaving, lowering, and reducing both of the mandibular angles. Shaping mandibular bone can be done by using ultrasound instruments which can:
• Make a much more precise cut
• Polish the jawbone more efficiently
• Not hurt or injured the soft tissues around the mandibular bone
 4. Closing the incision
After removing a certain amount of the bone, now it's time for closing the wound. Because in jaw reduction surgery the surgeon makes a cut inside your mouth, the stitches should be dissolvable.
5. End of the surgery
Your surgeon will place a surgical bandage over the incision to make recovery faster and easier. Sometimes the surgeon can combine chin feminization and jaw reduction surgery to achieve balance and beauty in your face. The surgeon may also recommend a face-lift or a neck lift for you if you have lost skin elasticity with aging.

Risks and complications

Like every other procedure, there may be some risks in jaw reduction surgery which include:

  • Anesthesia risks, if you have an allergy to specific anesthesia drugs, confirm with your surgeon about it.
  • Bleeding, during the jaw reduction surgery, the surgeon may accidentally cut a vessel so bleeding may happen.
  • Injury to the Inferior alveolar nerve may lead to numbness, damage to the lower lip, or death
  • Failure to reattach a muscle which name is mentalis muscle. This muscle elevates the chin and lower lip. When the surgeon can't reattach this muscle, this will cause a permanent problem.
  • Hematoma
  • Infection
  • Over-correction of the mandibular bone
  • Under-correction of the mandibular bone
  • Asymmetry
  • Swelling
  • Scarring: healthy tissues may be destroyed from jaw reduction surgery, so scar tissues take place of them. Scar tissues are thick and fibrous and are not always painful due to destroying nerves in this area. But after a while, they will be painful because the nerve endings can regenerate.
  • Sensitivity to the touch

Recovery

Immediately after the jaw reduction surgery, you may need to stay in the hospital for a while. After the discharge, you will leave the hospital with a bandage that covers your lower face. This bandage can be removed a couple of days later. Since jaw reduction surgery considers the most painful facial surgery, you may not be able to chew for several days. So you need to eat liquid foods during the first days. After a while, with your doctor's consult, you can follow a soft food diet.
Don't forget to take care of the wound hygienically because it's so important in the healing process and preventing infections. To achieve these goals you should brush your teeth and rinse your mouth with special liquid 3 times a day, one day after the surgery. Cold compresses can also help you a lot during the first day after surgery.
You can start again your usual activities after a week but if you want to begin heavy work or exercise you need to wait for the second or third week after the surgery. After a month all of the results appear and you will be more beautiful compares to before.
